I acknowledge that I have read and understood the SAIT Data Privacy Notice and that I am fully informed of the following:

• The types of personal data that will be collected and processed by SAIT, including employee profile and account credentials metadata.
• The purposes for which the data will be used, including employee authentication, attendance and workload systems, payroll and HR administration, internal reporting, and compliance requirements.
• The storage, retention, and eventual disposal of my personal data based on legal, regulatory, and institutional retention policies.
• The sharing of personal data with authorized internal units and lawful third parties only when required for operations, audits, or legal compliance.
• My rights as a data subject under the Data Privacy Act of 2012, including the right to be informed, access, correct, object, data portability, erasure/blocking, damages, file a complaint, and transmissibility.
• The contact information of SAIT's Data Protection Officer for any queries or concerns.

I understand that I have the right to withhold consent, but acknowledge that doing so may affect my access to SAIT employee systems and my participation in required institutional processes.

By signing this consent form, I voluntarily and freely give SAIT permission to process my personal data for the purposes stated above and in accordance with its Data Privacy Notice.
